● Packet #1: TTV Method Information 

● Packet #2: History & Consent

● Parent/Guardian Release Form ( if under 18)

Be sure to fill out all highlighted sections, including signatures and dates.

When you're ready, attach the following to the patient request form: 

● Clear photos of the tattoo and placement area

● Completed History & Consent (Packet #2)

● Parent/Guardian Release Form (if needed)
